Becoming a member

Membership in our practice is governed by the membership agreement you sign when you enroll, not by this page. That agreement covers what your membership includes, what it costs, how billing works, and how to cancel. If anything on this website appears to conflict with your membership agreement, the agreement is what applies.

Prices, services, and availability described on this site may change. We update the site when they do, but it is not a contract and it is not an offer.

Contacting us through this site

You may use our forms, phone number, and email to ask about becoming a patient or to reach our team.

Please do not send detailed medical information through a website form or ordinary email. Those channels are not secure. Once you are a member, we will show you the right way to reach your provider with medical questions.

Scheduling

All visits are scheduled. We ask that you call or text before coming in so we can confirm a time and prepare for your visit. We do not accept walk-ins.

Our content

The text, images, logo, and design on this site belong to Gem MD or are used with permission. You are welcome to read, print, and share pages for your own personal use. Please do not republish, reproduce, or use them commercially without asking us first.

Links to other sites

Some pages link to third parties, including our enrollment portal and outside health resources. We do not control those sites and are not responsible for their content or their privacy practices. Their terms apply when you visit them.
