About Gem MD
Gem MD is a Direct Primary Care practice in Rockwall, Texas. Dr. Eva Mackey Meyrat opened it in 2017 after years in insurance-based family medicine, and the model is the whole point: patients pay a flat monthly membership, we bill no insurance, and nobody on this team spends their day on coding, claims or prior authorizations.
What that buys is time. Members get unlimited office visits, unlimited phone, email and text access to their provider, and no charge for tests we run in the office. Visits are scheduled rather than squeezed in, and the practice does not take walk-ins.
The team today is Dr. Meyrat, two family nurse practitioners and a nurse who runs the practice's virtual support. The practice holds a 5.0 rating across 55 Google reviews.
The role
We are looking for a board-certified family medicine physician to join the practice and build a panel.
This is outpatient family medicine across all ages, with no obstetrics. Alongside primary care, the practice runs a medically supervised weight management program using GLP-1 medication, women's health, chronic disease management, same-day acute care, in-office labs and IV hydration. You would practice the full breadth of it.
What a day actually looks like
Office hours are Monday through Friday, 8:30am to 4:30pm, at a single location on North Lakeshore Drive.
Because there is no insurance billing, there is no billing department, no RVU target and no coding review. Appointments are long enough to finish the conversation. Patients reach their provider directly between visits rather than through a call center, which changes the shape of a day: fewer visits, more continuity, and far fewer loose ends carried home.
Who this suits
The physicians who do well in this model tend to be the ones who are tired of the one they are in.
The published research is consistent on this. Direct Primary Care physicians report 94% satisfaction with their practice against 57% among employed physicians, and 49% report zero burnout against 14%. Traditional primary care panels run two to three thousand patients per physician; a DPC panel runs 200 to 600. That difference is why the model exists.
If you are considering leaving an employed role, or you have thought about independent practice but do not want to build the administrative side of it yourself, this is the conversation worth having.
Compensation and structure
Compensation and schedule are discussed directly with Dr. Meyrat. The practice is open about the fact that a Direct Primary Care role is structured differently from an employed hospital position, and the right structure depends on where you are in your career and what you want to build. That is a conversation, not a line in a posting.
